File format for md5($pass.$salt)

Hi!

I'd like to use to hash type "MD5_PS" (i have the salt key and the final hash).
The following wiki page indicates that this is supported (http://www.cryptohaze.com/wiki/index.php/Cryptohaze_Multiforcer_Supported_Hashes#MD5_SP_-_md5.28.24salt..24pass.29), but it doesn't give the file format you should use ("I honestly forget what the file format is, so I'll fill this out later." on the wiki :( )

Any idea of the file format I should provide to Multiforcers to indicate both hash and salt key?
